APPROVED
Work with the instructor to fill out the Workshop Contract PDF and the W-9 PDF before the workshop. Scroll down for the PDFs
BEFORE YOU START
The board needs the cost and pricing info, and the form asks for that information as well.
Once you’ve filled out this form, you’ll work with the board for approval and to pick dates. The final step is to work with the instructor to fill out the workshop contract and to obtain a W-9 form from the instructor BEFORE the workshop starts.
