The submission process for Reflections requires that each artist fill out TWO (2) forms. Both forms are due no later than 7/12/2021.
PVA requires each artist fill out and agree to the terms of their loan agreement as part of the submission process. You must fill out the PVA Loan Agreement form at this link AND you must fill out the form below.

You must fill this form out in it’s entirety in one session or the data you have entered will be lost if you log out (or are timed out).

If you are unable to submit the forms electronically via the website form below, there are PDF versions of the form that you can print and fill out after printing. Note that you will then need to photograph or scan the PVA Loan Agreement and email that to Hedwig@pvarts.org. You will also need to email one sheet of the Labels and Records form to exhibits@mbmag.org There are also fillable PDFs you may request.
